A Comparison of the Efficacy between Traditional Surgical-Hand Scrubbing and Brushless-Hand Rub with Alcohol-based agent

Abstract
This study was propective clinical trial which has the purpose to compare the efficacy between Traditional Surgical hand Scrub(TSS) and the Brushless Hand Rub(BHR) in terms of antimicrobial effectiveness, effect on skin condition, easy-to-use and cost. The subjects were 26 OR staff nurses in two army hospitals. The differences of the counts of the Colony-Forming Units(CFUs) at each time interval, immediately and 3-hour after the application, between the two regimens were insignificant, while the CFUs counts were reduced significantly from baseline microbial counts at each time respectively. All the TSS¡¯s skin condition is significantly worse than BHR¡¯s. Especially subjects reported the highest score(5.60¡¾2.78) on dryness of TSS, which means the worst condition among six skin condition items. Easy-to-use score of BHR was significantly higher than TSS (t=6.56, p<.001). This meaned that BHR was more convenient for use than TSS Product¡¯s price of BHR is more expensive than TSS, but TSS need more time and resources. The sum of product¡¯s price and cost of time required is BHR 17,777 won vs 60,492 won. Besides, TSS required 4.55L per one time, additional povidone solution, reusable sterile towel and more emollient. Even worse, single used item(i.e. brush and package) was produced every TSS In conclusion, BHR significantly reduced microbial counts on hands, improved skin condition, and was less costly.
