Modified MMA approach for Asian with obstructive sleep apnea syndrome - a case report

Abstract
Maxillomandibular advancement(MMA) is a treatment for obstructive sleep apnea syndrome(OSAS) that enlarges the pharyngeal and hypopharyngeal space by altering the facial skeletal framework resulting in less pharyngeal collapse during negative-pressure inspiration. In Caucasian patients, an average of 9.5mm or at least 10~12mm of advancement has been recommended to achieve maximum airway expansion. But in Asian patients, this amount of advancement may be esthetically unfavorable since the facial profile is less convex, upper and lower lips more protrusive, and nasal structure smaller. The purpose of this study was to introduce the modified MMA surgical treatment, MMA combined with bimaxillary anterior segmental osteotomy(ASO), for Asian OSAS patients thereby solving both functional and esthetical problems.
